1. Launch OffiSign-In
  2. Click on the Help menu at the top
  3. Expand the Logs sub-menu
  4. Click on Enable Log Collection and then click Yes in the subsequent dialog box
  5. Restart OffiSign-In
  6. Reproduce the problem you are having (e.g. trigger the error, sign in a patient)
  7. Send the logs

Make sure to disable the extra logging when your problem is resolved!
